What Is the Best Treatment for Vertical Lip Lines and Mouth Wrinkles?

What Is the Best Treatment for Vertical Lip Lines and Mouth Wrinkles?

Written and medically reviewed by Sean E. Mazloom, MD, FAAD — Double Board-Certified Dermatologist, Cosmetic and Laser Expert · Oct 5, 2026

The short answer

There is no single “best” treatment for vertical lip lines. Because perioral aging reflects a combination of repetitive orbicularis oris movement, collagen loss and photodamage, and loss of structural support, the strongest strategy is usually anatomy-driven combination treatment rather than relying on one modality alone. For most patients, that means addressing three complementary targets:

  • Laser resurfacing or RF microneedling to stimulate collagen and smooth etched-in lines. Ablative CO2 provides the most aggressive resurfacing for deep static rhytides, while fractional approaches and RF microneedling can offer meaningful improvement with shorter recovery.
  • Hyaluronic acid filler to restore lost lip volume and support the skin under the lines.
  • Botox in small doses to relax the lip muscle that keeps creasing the skin.

At REJUVA Dermatology, Vein & Cosmetic Center in Venice, Florida, we build each plan around how deep your lines are, your skin type, and how much downtime you can accept.

Why lip lines form — and why it changes the treatment

Vertical lines above and below the lips are often called smoker’s lines, lipstick lines, or barcode lines. You do not have to smoke to get them. They form from three causes working together:

  1. Muscle movement. The orbicularis oris muscle circles your mouth. Every time you purse your lips, drink from a straw, or talk, it folds the skin in the same spots.
  2. Aging and sun damage. Over time, collagen and elastin break down. Florida sun speeds this up. Lines that once appeared only with movement become etched in at rest.
  3. Volume loss. The lips and the skin around them thin and flatten with age. The corners of the mouth can turn down, and the lip border loses definition.

Each cause responds to a different treatment. Deep static etched lines respond primarily to resurfacing and collagen remodeling; volume-related flattening and loss of lip support respond to filler; and dynamic creasing from repetitive muscle activity can respond to carefully selected neurotoxin treatment. This is why combination therapy is often more complete than monotherapy.

Your treatment options, explained

CO2 laser resurfacing: the gold standard for deep lines

CO2 laser resurfacing remains a reference treatment for deep vertical perioral rhytides because it can provide substantial ablation and collagen remodeling, addressing etched lines that filler alone cannot fully correct.

Fully confluent ablative CO2 is now used more selectively, but the perioral and periorbital regions remain areas where aggressive resurfacing may be considered when maximal wrinkle effacement is the goal. The trade-off is substantially greater downtime, prolonged erythema, anesthesia requirements, and higher risks of dyspigmentation or scarring—particularly in darker phototypes.

Fractional CO2 creates microscopic treatment columns while preserving intervening tissue, accelerating re-epithelialization compared with confluent ablation. Makeup may often be resumed in roughly 4–7 days, although multiple sessions may be needed to approach the degree of correction achievable with more aggressive confluent resurfacing.

Dual Frax: an option for patients searching for Fraxel Duo® with less downtime

Patients searching online for Fraxel Duo® or Fraxel DUAL® are generally looking for dual-wavelength fractional resurfacing using 1927 nm and 1550 nm wavelengths. At REJUVA, we offer Dual Frax, a newer-generation dual-wavelength fractional laser treatment with additional features and treatment capabilities that allow greater customization. Its 1927 nm wavelength targets more superficial photodamage, discoloration, and texture, while the 1550 nm wavelength reaches deeper for collagen remodeling. Published literature on nonablative and hybrid fractional resurfacing supports this general strategy for patients prioritizing less downtime, although deeper etched rhytides may require more aggressive resurfacing.

Because Dual Frax is non-ablative, recovery is generally shorter than with ablative CO2 resurfacing. Patients commonly experience temporary redness, swelling, and a sandpaper-like texture as the skin renews. For fine to moderate vertical lip lines—especially when sun damage or uneven texture is also present—it can be an attractive option for patients who want collagen remodeling with less downtime than aggressive CO2 resurfacing. [6,8,9]

Who it fits best: patients with fine to moderate lip lines, sun damage, and texture concerns who prefer less downtime. Deep, etched barcode lines may require more aggressive resurfacing or a combination plan. Dual Frax can also be paired strategically with hyaluronic acid filler and small-dose Botox when volume loss and repetitive lip movement contribute to the lines.

RF microneedling: strong results for all skin types

Radiofrequency (RF) microneedling delivers RF energy into the dermis through microneedles, stimulating collagen remodeling and tightening while largely sparing the epidermis. This makes it useful across a broad range of skin tones, with typical recovery consisting of transient erythema and edema. [7,10]

Clinical evidence for perioral RF microneedling is encouraging. A pilot study using four monthly treatments reported significant improvement in upper-lip, lower-lip, and oral-commissure lines at six months, with objective improvements in dermal density and elasticity and no reported dyspigmentation or scarring. A randomized split-face study comparing RF microneedling with fractional ablative 2940-nm Er:YAG found comparable clinical rejuvenation outcomes, with RF requiring multiple visits and causing more procedural discomfort, while laser produced more erythema and longer recovery.

Hyaluronic acid filler: restoring volume and support

Hyaluronic acid (HA) is the predominant filler category for lip and perioral rejuvenation. A systematic review of FDA-approved filler materials found HA to be particularly well suited to this region, with high satisfaction and a favorable complication profile.

Filler addresses what resurfacing cannot: lip and perioral volume loss, Cupid’s bow and vermilion-border definition, oral-commissure descent, and structural support beneath fine lines. Small-particle HA fillers have also been studied specifically for correction of perioral rhytides.

Technique and anatomy are paramount. Perioral correction may use soft, appropriately selected HA placed with precise superficial techniques, with careful attention to the facial and labial arterial anatomy to reduce vascular risk.

Botox: softening the lines that come from movement

Small doses of botulinum toxin can soften the repetitive orbicularis oris activity that contributes to dynamic vertical lip lines. It is frequently combined with filler and resurfacing when muscle activity is one component of the patient’s perioral aging pattern.

Which plan is right for you?

Your plan depends on what is driving your lines and what your schedule allows. Here is how we typically match treatment to patient:

  • Deep, etched lines and you can take time off: CO2 or fractional CO2 laser for the most dramatic smoothing.
  • Fine to moderate lines and you need to be back to work quickly: Dual Frax, often combined with filler and Botox.
  • Darker skin tone, or you want minimal downtime with strong collagen building: RF microneedling.
  • Thin lips, flat lip border, or down-turned corners: hyaluronic acid filler.
  • Lines that deepen when you talk or purse your lips: Botox.

Most patients benefit from addressing more than one contributor. A common strategy pairs resurfacing or collagen stimulation with filler for structural support and, when appropriate, small-dose botulinum toxin for dynamic creasing.

Frequently asked questions

What is the best treatment for smoker’s lines?

There is no single best treatment for every patient. The most complete correction usually comes from matching treatment to the causes of the lines: resurfacing or RF microneedling for etched texture and collagen loss, hyaluronic acid filler for structural support and volume loss, and carefully selected botulinum toxin for dynamic muscle-related creasing. Deep static lines may benefit most from CO2 resurfacing. [1–7]

How is Dual Frax similar to Fraxel Duo®?

Dual Frax and Fraxel Duo® are similar in treatment concept because both use 1927 nm and 1550 nm fractional wavelengths to address different depths of the skin. These wavelengths can target concerns such as photodamage, uneven pigmentation, rough texture, fine lines, and deeper collagen remodeling.

At REJUVA, we use Dual Frax, a newer-generation dual-wavelength fractional laser technology with additional treatment features and capabilities that allow us to customize resurfacing based on a patient’s skin concerns, treatment goals, and desired downtime. Patients searching for Fraxel Duo® or Fraxel DUAL® are often looking for this type of advanced non-ablative fractional resurfacing.

For vertical lip lines and wrinkles around the mouth, we evaluate whether Dual Frax alone—or a customized combination with CO2 laser resurfacing, RF microneedling, filler, or Botox—will provide the best approach for the patient’s particular pattern of aging and desired recovery time.

Can lip filler alone fix vertical lip lines?

Filler helps, especially when volume loss is part of the problem. Etched lines in the skin usually need resurfacing as well. Lines caused by movement return without Botox. Combining treatments gives the most complete, longest-lasting result.

How much downtime should I expect?

It depends on the treatment. Botox has essentially no recovery time for most patients. Filler may cause brief swelling or bruising. Dual Frax and RF microneedling typically involve a shorter recovery than ablative resurfacing, while fractional CO2 commonly requires several days and fully ablative CO2 can involve weeks of visible erythema.

How many treatments will I need?

Fully ablative CO2 is often a single treatment. Fractional CO2 usually takes 2 to 3 sessions, and RF microneedling about 3 to 4. Dual Frax is done as a series tailored to your goals. Filler and Botox are maintained over time.

Is lip line treatment safe for darker skin?

Yes, with the right choice of treatment. RF microneedling is particularly useful when minimizing epidermal injury is important and has been studied across a broad range of skin types. Aggressive ablative resurfacing carries a higher risk of pigment alteration in darker phototypes, so treatment selection and settings should be individualized.

Does insurance cover treatment for lip lines?

No. Treatment for lip lines is cosmetic and is not covered by insurance. We review costs with you during your consultation.

How long do results last?

Collagen remodeling from laser and RF treatments can provide durable improvement, although intrinsic aging and sun exposure continue. HA filler and botulinum toxin require maintenance over time. Longevity varies by product, treatment intensity, anatomy, and individual response.
