Melasma is a chronic, acquired and persistent skin condition consisting of increased hyperpigmentation on the skin. The sun-exposed areas of the face such as the forehead, cheeks, upper lip, and chin are most commonly affected. This condition is more common in darker female patients such as Asian, Hispanic, and Middle Eastern patients.
Melasma causes include sun exposure (UV radiation), genetics, female sex, high levels of estrogens (hyperestrogenism) such as during pregnancy or due to contraceptive medications, thyroid disorders, and certain medications can all predispose a person to melasma. The pigmentation occurs due to an increased activity of melanocytes (pigment cells) which in turn increases the amount of melanin production.
Melasma symptoms appear on the skin as irregular patches of darker brown spots. These darker areas are known as “hyperpigmentation” of the skin, and may have a darker gray to brown color. The borders of pigmentation are irregular but well-demarcated. It is common to have more than one patch of hyperpigmentation melasma on the face. This condition is more common in females and rarely ever seen in males.
There are several treatment options for melasma that vary based on the patient, severity, distribution of patches and skin tone. Melasma is a chronic and persistent condition that is difficult to treat. Therefore, familiarity with this condition and its treatments are required for good clinical outcome.
