Stasis Dermatitis is a type of skin rash that occurs on the legs due to poor blood return to the heart. The underlying mechanism is vascular in origin.
The underlying cause of Stasis Dermatitis is venous insufficiency. There are one-way valves inside our veins that function to help return deoxygenated blood to the heart when working properly. Venous insufficiency occurs when the valves inside of our veins become incompetent and start to weaken which allows blood to backflow and pool in the bottom of our legs due to gravity. The veins start to become leaky due to fluid overload and as a result overlying skin changes known as Stasis Dermatitis occur. Not everyone that has venous insufficiency develops the condition but it does place individuals at a much higher risk. Other risk factors include being female, overweight, living a sedentary lifestyle, increasing age, prior trauma, injury, surgery or blood clot affecting the vasculature of the legs.
Venous Stasis Dermatitis is characterized by lower leg swelling especially after prolonged periods of standing, darkly pigmented skin discoloration, scaling, skin hardening, and intensely itchy skin. Over time ulcers and open wounds can form and shrinkage or narrowing of the lower calf may occur giving the lower legs an “upside down bowling pin” appearance termed lipodermatosclerosis. Patients may exhibit other symptoms of venous insufficiency such as leg pain or cramping, the presence of varicose veins or spider veins, edema that is worse at the end of the day, and a sensation of heaviness or aching in one or both legs.
Treatment of this condition is aimed at both alieving current symptoms and addressing the underlying cause. Topical steroids may be prescribed to help improve the rash and itchiness and reduce skin inflammation. Supportive measures include wearing medical-grade compression stockings, elevating the feet while sitting, and engaging in regular physical activity. Wound care should be applied to any ulcers or sores present. Oral antibiotics may be warranted if a secondary skin infection results. Frequent moisturization of the legs will help repair and maintain a healthy skin barrier.
